When Should I Use This Form?

The various medical forms available from BSA are used to record important medical history that might be needed in the event that a Scout or Leader is injured. In addition, the forms may give permission for a person to participate in certain types of activities and to be treated if injured while a parent or guardian is not immediately available.

Since there are three different forms (Class 1, Class 2, and Class 3) which cover Cub Scouts, Boy Scouts, and Leaders for various types of activities, figuring out which form to use sometimes becomes confusing. Hopefully, the following explanation will help to demystify the topic:

  1. Are you an adult leader 40 years of age or older?
    • YES - If you plan on participating in any physical activity, you must have a current CLASS 3 medical form on file with your unit. STOP RIGHT HERE!
    • NO - Go on to step 2

  2. Since you are under 40, you must have a current CLASS 1 medical form on file with your unit.
    Are you planning on participating in high adventure, athletic competition, or attending a world jamboree?
    • YES - You must also have a current CLASS 3 medical form on file with your unit. Go on to step 3.
    • NO - Go on to step 3.

  3. Are you planning on attending any activity lasting 72 consecutive hours or longer (e.g., resident summer camp, backpacking trips, etc)
    • YES - You must also have a current CLASS 2 medical form on file with your unit.
    • NO - All done!

Typically, Cub Scouts use Class 1 forms (which are printed on the back of the Cub Scout application) and Cub Scout leaders would complete Class 1 or Class 3 forms, based upon their age.

Class 1 forms must be renewed every year and do not require a doctor's visit.

Class 2 forms must be renewed every 3 years and require a doctor's visit.

Class 3 forms must be renewed every year and require a doctor's visit.
